A risk assessment is an immediate, evidence-based assessment made by a mental health professional or physician to assess an individual who may be experiencing a mental health crisis. Risk assessment locations (listed below) are where you can take your student for an immediate risk assessment, referred to or required by South Central Community School Corporation.

You may call ahead to communicate with the facility that you and your student will be arriving. Please bring the documentation given by the school to the facility. Before the student can return to school, you must please provide a letter from the facility stating that they are medically cleared to return. If the student has been recommended for further intensive services, please provide the school with the documentation of the physician's recommendation.
